Recognition Request
The City receives many requests for ceremonial items (for example, commendations and proclamations) from various entities and residents.
These types of requests are awarded at the discretion of the City. To submit your request, please use the request form. Please submit your request three weeks before your requested/required date. The following is a brief explanation of the various types of requests and criteria.
Proclamation
Purpose: Generally proclaims certain periods of time (days, weeks or months) in honor of an event or activity promoting health, welfare, heritage of the community (e.g., American Red Cross Month, Ride to Work Week, Arbor/Earth Day).

Certificates
Purpose: To express appreciation for exemplary service rendered to the City of Thousand Oaks when plaques, commendations, proclamations, etc. are not appropriate due to number of persons or the fact that some individuals have already received commendations (e.g., CVUSD retiring teachers, Elks Lodge, Fire, Police, and Honorees).
Awarded to: Individuals

Special Presentations at City Council Meeting
Purpose: To present any type of the above types of recognitions during a City Council meeting. To present the City with an award (e.g. recognition). To present introductory performance for events at the Civic Arts Plaza.
